If a certificate issued by our institution is lost or if you wish to request a new certificate, please deposit an amount of Rs. 850.00 to Account No. 7442918 at Bank of Ceylon, Homagama Branch, and send a written request including the following details:
- Full name to be included on the certificate
- National Identity Card Number (ID No.)
- If requesting a correction, the currently issued certificate
- Contact telephone number
- Original receipt of the Rs. 850.00 payment
